Research Scanning Forms

When scanning participants you will need to complete the Participant Information Sheet, Consent Form, and Safety Screening Form. You will also need to include these with your Ethics Application.​

If you are an external researcher and do not have access to MyUEA, please contact Mark Terry, or Will Penny to request these forms.
