Raising Concerns and Reporting Incidents Relating to Practice
You can raise a 'concern' when you are worried about an issue that affects the people you are caring for and you are acting to protect them. This form can also be used to notify the School of Health Sciences of any incidents which have occurred in practice which involve HSC learners, or if an incident has impacted on the practice area as a learning environment. -
Raising an issue of concern regarding a student
This form can be completed by any member of the University (academic, administrative or support staff), a member of staff from a practise placement area, a member of the public or another student. -
Raising a Concern guidance
If the concern is being raised by the practice placement environment, the Link Lecturer/Visiting Tutor should be made aware that a concern has been raised whenever possible. The concern form should normally only be completed if there issues which sit outside of the normal assessment processes. -
