Module Overview / Introduction
The purpose of this module is to support practitioners in promoting person-centred care for the patient and their families/carers with a long-term condition. It will enable practitioners to understand the challenges and complexities of living with a long-term condition and the strategies and interventions used to support people to self -manage their illness. It will explore emerging models of service provision for the patient and provide opportunities for enhanced collaboration between services in the health and social care sectors, also across primary, secondary and tertiary care. Central to this module is the significance of using evidence-based approaches to manage long-term conditions and to understand this in the context of contemporary health and social care practice. Therefore, the practitioner is encouraged to reflect on their practice and continuously be an advocate for driving and improving service provision within their own area of practice. Working with people with long term conditions is essential in this module. There is an expectation that you attend and participate in all live online sessions.

Learning Outcomes
- Critically appraise current research/evidence/policy and their relevant application to practice in relation to patients with long-term conditions
- Explore approaches and strategies to partnership working with patients to enable informed patient choice and involvement
- Develop critical awareness of the effects of life transitions and illness / disability on the patient, their families / carers and on the practitioner- patient
- Critically appreciate a range of models of working with service users, expert patients and carers
- Develop critical awareness of the psychosocial effects of long-term conditions on the patient, their families and carers.
- Explore effective skills/approaches in the management of the patient journey considering all aspects of the generic long conditions model such as case management, personalised care plans and self-management.
- Analyse and evaluate skills in assessing, prioritising, planning & evaluating care & leading health education & health promotion strategies in the management of long -term conditions
